Forms & Documents
Your providers may assign forms for you to complete — such as intake questionnaires, consent documents, or health assessments.
Viewing assigned forms
Section titled “Viewing assigned forms”- Click Forms in the sidebar
- Your assigned forms are listed with their status and due date
- Forms may be marked as Pending, Overdue, or Completed
Completing a form
Section titled “Completing a form”- Click on a pending form to open it
- Fill in the required fields — required fields are marked with an indicator
- Some forms have multiple pages — use the navigation to move between pages
- Review your answers before submitting
- Click Submit to send the completed form to your provider
Form types
Section titled “Form types”| Type | Purpose |

| Intake | Gather your health history and personal information |
| Assessment | Evaluate your current wellness or specific conditions |
| Questionnaire | Collect feedback or preferences |
| Consent | Agreements and consent forms for treatment |
Due dates
Section titled “Due dates”Some forms have a due date set by your provider. Forms that are past their due date are marked as Overdue. Complete overdue forms as soon as possible.
Uploaded documents
Section titled “Uploaded documents”Your provider may also share documents with you (such as PDFs). These appear in your Forms section and can be downloaded for your records.
Troubleshooting
Section titled “Troubleshooting”I can’t access an assigned form
Section titled “I can’t access an assigned form”Make sure you have an active connection with the provider who assigned the form. If your relationship status has changed, the form may no longer be accessible.
I submitted the form but it still shows as pending
Section titled “I submitted the form but it still shows as pending”Form submission status updates in real time. Try refreshing the page. If the issue persists, contact your provider.
